Macmillan Specialist Care at Home in Dudley aims to transform the way palliative care and end of life care services are provided for people affected by cancer & other life limiting conditions.
The service aims to maximise patient choice by providing as much treatment and support in the home or community as possible. Hospital, hospice & community teams working closely together, resulting in care that is better co-ordinated and puts the needs of the patient first.
We do this by working with teams from different disciplines in the community to enable patients and carers access to specialist care at home without the need to attend hospital or be admitted.
Based at Mary Stevens Hospice, our hub makes it quicker and easier for health professionals to refer patients to the specialist palliative care team in Dudley. A single number takes you straight through to the Dudley Macmillan Specialist Care at Home team. Referrals are assessed as they are received and passed to the most appropriate person in the team based on need and urgency. In addition an advice line is available Monday – Friday, 9am – 5pm, manned by a clinician to provide telephone support to professionals who require specialist palliative care advice.
